Why do I need to sign an authorization?
The HIPAA Privacy Rule requires us to obtain your authorization for uses and disclosures of protected health information not otherwise allowed by the HIPAA Privacy Rule, such as for treatment, payment, and health care operations.
When will my authorization expire?
This authorization does not expire until it is revoked by you. After the expiration of this authorization, Progyny will not use or disclose your health information for the purposes described herein, unless you authorize such additional use or disclosure by submitting another authorization.
Can my information be redisclosed?
Yes, your information may be re-disclosed if the recipient(s) described in the authorization is not required by law to protect the privacy of the information, and such information is no longer protected by federal health information privacy regulations.
Can I refuse to sign this authorization?
Yes, you have a right to refuse to agree to the terms of this authorization. Your health care, the payment for your health care, and your health care benefits will not be affected if you do not agree to the terms of this authorization, but we will not be permitted to disclose your information as described on this authorization without your agreement.
How do I obtain a copy of my signed authorization?
You have a right to receive a copy of this signed authorization. If you would like a copy of this authorization, please send your request to:

Can I revoke my authorization?
Yes, you have the right to revoke your authorization at any time, except to the extent that Progyny has already taken action based upon your authorization. To revoke this authorization, please write to:

How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services. To file a complaint with us, please contact the Privacy Officer at 646.370.0747 or send a letter to us to the attention of the Privacy Officer. No one will retaliate or take action against you for filing a complaint.
